Constant phlegm in throat?!?

Alright so for as long as I can remember I have had phlegm in my throat making me talk strange occasionally because it is so thick and disgusting, it even makes it hard to breathe because it is so thick. So my problem is I constantly clear my throat the second I feel it. My family is annoyed about it and it affects my decisions because I'm worried about even getting a job if people will hear it and be annoyed! Help?! Water just makes it THIN it's still there though
